Medical Records

Medical records can be obtained at any time by going to your patient portal account, selecting the appropriate patient account, navigating to “My Chart”, and selecting either “Print My Chart” or “Download My Chart”. If you do not already have an active portal account, please visit the “Patient Portal” page on our website for instructions on how to obtain one.
